Referral Procedure By appointment. Persons identified by Community Health Nurses of having risk factors for non-communicable diseases such as diabetes, hypertension (high blood pressure, stroke, and cardiovascular disease.

About Adult Preventive Care Clinic (The)

Purpose

The Adult Preventative Care Clinic provides health education and support to persons who are risk for Diabetes, Hypertension, Stroke and Cardiac Disease. Seen by appointments only.

Services

  • Basic screenings, health education and promotion, immunisations, nutrition assessments/counselling and counselling, and functional assessments.
