About Community Health Nursing
Purpose
Provides primary care nursing in the home and arranges for other support services for patients, care-givers and the general public.
Services
- Nurses provide:
Health education - Health promotion and education
- Wound care, injections, vaccinations including flu shots and COVID 19 boosters for homebound persons.
- Medication support
- Adult immunizations
- · Adult Health Clinics
- Flu Express
- Take it to the Streets
- Adult Health Clinics- Appointment only on Wednesdays- serves the under and uninsured persons which allow access to medical care, including complete physicals and regular follow up appointments.
- Personal Care Services (bathing etc.) are provided short term by our Community Health Workers (Nurses Assistants).
